Veterans: Click on the “Veterans” application form.  Once it opens, print it, fill it out and mail back to the address at bottom of page.  Your application will be reviewed and filed for the next Honor Flight trip.  You will be contacted by mail informing you a few weeks before the trip if you are accepted or you will have alternate status.  Veterans Application

Guardians: Click on the “Guardian” application  form.  Once it opens, save a copy to your PC. You can then fill out the form and email a copy to: info@texassouthplainshonorflight.org . Alternatively you can fill out the form, print it, and mail it to the TSPHF address found on the last page of the form. Your application will be reviewed by the TSPHF committee. Once it is determined that your veteran is accepted, you will be informed that your staus is also accepted.  You will be required  to mail in your check to cover guardian costs at that time, so be prepared!  Guardian Application.
